The moment you walk in the door at Niswander Eye Center, you’re warmly greeted, and treated as an individual. We cater to your specific needs as a patient. Our ophthalmologists, optometrists and opticians treat your vision as your greatest asset. Your comfort and eyesight are held in the highest regard before, during and after your eye exam or surgery.

As a patient at Niswander Eye Center, there are a few things we would like you to know. Please refer to the following exam preparation tips and forms before your scheduled appointment. We make it easy to pay for your procedures by accepting most major credit cards, insurance plans and offering helpful financing options.

Prior to your appointment at Niswander Eye Center, please download and complete the necessary forms.

New patients should read our welcome letter and complete the Patient Registration and both pages of the Medical History form. Please bring the completed forms with you to your appointment.

In the case of an emergency, you may call Niswander Eye Center at any time. If your emergency takes place outside of regular business hours, you’ll be directed to our answering service. They will notify the on-call physician of your emergency.

We respectfully request that patients do not call the office outside of regular business hours with non-emergent questions or concerns, or for prescription refills.

Parents & Guardians

Patients under the age of 18 must be accompanied to all appointments by a parent or guardian. If the parent or guardian is unable to accompany the child, please fax the completed Authorization to Examine a Child form to 716.634.3174 or send it along at the time of your child’s exam.



Niswander Eye Center accepts most insurance plans. Copayments and deductibles are due at the time of service.

No Show Policy

A "No Show" is the term we use when a patient misses an appointment without cancelling it within one (1) business day. 

Failure to show  at the time of a scheduled appointment will be recorded in your medical record as a No Show. A fee of $35 will be charged to your account. A letter and a bill will be sent to you alerting you to the fact that you missed an appointment, and failed to cancel it within one (1) business day. Three No Show's in one calendar year may result in suspension of services. The No Show charges are patient responsibility and will not be billed to your insurance company.
